Reflux diagnosis

When diagnosing of gastroesophageal reflux disease, one or more of the following tests may be recommended.

• Gastroscopy: biopsies can be performed during gastroscopy
• Esophageal pH monitoring
• Esophageal manometry
• Barium swallow X-ray
• Scintigraphy

Making changes in lifestyle

One of the very first steps health care professionals counsels their patients during acid reflux treatment are lifestyle changes, like quitting smoking, abstaining from alcoholic beverages, eating less but more often etc

Drugs treatment

A drug which reduces the amount of acid made in your stomach is a common treatment and usually works well. But medications doesn’t treat the mechanical disorder which is origin of the reflux. Therefore, %90 of the patients who leaves the drug treatment symptoms begins again within a short period of time.

Reflux Surgery

Actual treatment can only be achieved with surgery. Randomized prospective controls in 10 years period shows the %93 success of surgical treatment.%93 of the patients who prefers the surgical treatment never have the same symptoms again and no longer required the use of drugs. The success rate of the surgery is directly related with the experiences of the surgery team.

Laparoscopic Reflux Surgery

Laparoscopic reflux surgery is regarded as a gold standard in the treatment of gastroesophageal reflux. It offers good functional results and high long-term quality of life. About half of the patients with severe Reflux Disease often have a hiatal hernia, which is a tear in the diaphragm.

Surgery is the primary option for reflux patients that also have a stomach hernia. In a laparoscopic procedure, surgeons use small incisions (1/4 to 1/2 inch) to enter the abdomen through cannulas (narrow tube-like instruments). The laparoscope, which is connected to a tiny video camera, is inserted through the small incision, giving the surgeon a magnified view of the patient's internal organs on a screen. If the patient have a hiatal hernia firstly needs repair of hernia. To reduce the number of potential postoperative recurrence our surgery team uses prothetic mesh.

The average duration of Laparoscopic Reflux surgery team, leading by Md. Levent Eminoglu is 35- 45 minutes. After surgery, patients stay at the clinic overnight and are usually released the next day, provided they feel fit enough.

Robotic Reflux Surgery

Many surgical interventions are performed laparoscopically, some technical specifications of laparoscopy bring out some limitations too. The starting point of robotic surgery was getting rid of these limitations. In particular, with the most sophisticated version of robotic surgery used in our hospital, known as SI Technology, it is possible to operate with 3 dimensional and high resolution images which are not available in laparoscopic surgery. When needed, this image can be magnified ten times, thus the operation zone can be viewed very closely and in detail, making it possible to operate with this image. Thanks to the improved interface of the da Vinci SI model, the hand movements made by the surgeon on the console can be reflected onto minute surgical equipment, and the shaking and fatigue, the result of long operating hours, are becoming a thing of the past.

Having made over 1500 laparoscopic acid reflux operations, surgeon Levent Eminoğlu and his team are one of the most experienced teams in Europe and Turkey on this subject and they perform robotic acid reflux surgery by penetrating from one single point in the abdomen. This means that pain after surgery and movement limitation decreases significantly. Also, especially for woman patients, by keeping by the surgery scar inside the belly button, it is possible for the surgery scar to vanish completely over time.

Endoscopic Reflux Treatment

Very recently, endoscopic techniques for the treatment of Reflux have been developed and tested. This non-invasive treatment is suitable for patients who are not required surgery.

Pilcator technology is the most popular treatment in last 5 years. In Turkey it is firstly performed in KadikoySifa Heath Group’s Rexlux center.

The Plicator device is inserted through the mouth and down the esophagus with the help of an endoscope. The device grasps, folds, and fixates tissue with a pre-tied suture at the junction of the stomach and esophagus. This tightens the valve that acts as a natural barrier to gastric reflux.1 Patients receive intravenous conscious sedation during the 20- to 30-minute procedure and return home the same day.
